Attention Facilities/Agencies Applying for Multiple Members
With the introduction of the new membership fee structure on December 8, 2009, the renewal process will automatically determine the membership category for each renewing member. The membership category will be based on the number of members from an individual Facility/Agency who have renewed after the anniversary date of the introduction of the new membership fee structure, namely December 8 each year. All membership calculations are based upon the anniversary date of the introduction of the new membership fee structure.

The first member renewing after the anniversary date in any given year will be given a membership category of 'Individual' and charged the Indivdual member membership fee.
The second member renewing after the anniversary date in any given year will also be given a membership cateogory of 'Individual' and be charged the individual member membership fee.
The third member renewing after the anniversary date in any given year will be given a membership category of Facility/Agency Individual, be charged $86.00 for his/her membership fee (3 @ $100 each minus 2 @ $107), and the two previously renewed members after the anniversary date in any given year will be adjusted to a 'Facility/Agency Individual' membership.
The fourth renewing member after the anniversary date in any given year will be assigned a membership category of 'Facility/Agency Indivdual' and be charged $100 for the membership fee.
The fifth renewing member after the anniversary date in any given year will be assigned a membership category of 'Facility/Agency Member', be charged $100 for the membership fee, and the four previously renewed members after the anniversary date in any given year will be adjusted to a 'Facility/Agency Member' membership.
The sixth, seventh and eighth renewing members after the anniversary date in any given year will be assigned a membership category of 'Facility/Agency Member' and be charged $0 for the membership fee because the 'Facility/Agency Member' membership fee has already been paid by the first five renewing members.
The nineth, tenth, eleventh, etc. renewing members after the anniversary date in any given year will be assigned a membership category of 'Facility/Agency Member' and be charged $50 for the membership fee.

Additionally, each individual member must make their own application for membership renewal. The renewal process is based on each individual's Member Number and Password. Using the browser 'Back' button to enter multiple renewals is prohibitted.
